Event Registration

I hereby acknowledge and understand the nature of the activities provided at the Art and the Environment Summer Camp at the Museum of Art l Fort Collins. By signing this waiver, I confirm that I have read, understood, and agreed to the terms and conditions outlined below:

1. Assumption of Risk: I am aware that participation in the Art and the Environment Summer Camp involves certain inherent risks and hazards. These risks include, but are not limited to, accidents, injuries, and exposure to natural elements. I understand that the camp organizers and staff will take reasonable precautions to minimize these risks, but some risks are unavoidable. I voluntarily accept and assume all such risks associated with the camp activities for my child.
2. Medical Authorization: I authorize the camp organizers, their designated staff, or emergency medical personnel to administer first aid and/or seek medical treatment for my child, if necessary, in the event of an injury, illness, or medical emergency during the camp. I understand that the camp organizers will make reasonable efforts to contact me or the emergency contacts listed in the registration form in such situations.
3. Code of Conduct: I understand that my child is expected to follow the rules and guidelines set by the camp organizers and staff. I agree to instruct my child to behave in a respectful and responsible manner towards fellow campers, staff, equipment, and the environment. I acknowledge that failure to comply with the code of conduct may result in disciplinary actions, up to and including dismissal from the camp without refund.
4. Supervision and Liability: I acknowledge that the camp organizers and staff will exercise reasonable care and supervision over the camp activities. However, I understand that they cannot be held liable for any loss, damage, injury, or accident that may occur during the camp, except in cases of gross negligence or willful misconduct.
5. Photography and Media Release: I grant the camp organizers permission to use photographs, videos, and other media recordings taken during the camp for promotional and educational purposes, including but not limited to camp brochures, websites, social media, and news releases. I understand that my child's identity will not be disclosed without my explicit consent.
6. Snack Permission: I give permission for my child to participate in the snack provided by the camp organizers. I understand that the camp organizers will make reasonable efforts to accommodate any known allergies or dietary restrictions indicated during registration. However, I will inform the camp staff of any changes or updates to my child's dietary needs.
